This is your opportunity to provide feedback to your club about your experiences during the 2016/17 season. As a thank you for taking part, you will have the chance to be entered into a prize draw to win a number of great prizes.
Your feedback is important to both the EFL and its clubs and all results will be shared directly with your club to allow your opinions to be considered when planning for their future.
